Please print and fill out the below forms prior to your first treatment session or evaluation. Client contracts will be emailed to you to review.

Policies

Sessions range from 45 minutes to 1 hour. Accommodations through teletherapy may allow for 30 minute sessions. Family education/training or consultations may occur virtually or over the phone. Contracts that begin after 2/27/2022 will indicate that the last 10 minutes of sessions will be allocated to documentation as well as cleaning and parent conversation.
